Covid-19 Track & Trace Form

A new law has been introduced, making it compulsory for any fixed site premises, (pubs, restaurants, cafes etc.) to record customer details in order to support the NHS Test & Trace system.

To support you in keeping these records, we’ve created some handy Covid-19 Track & Trace forms for you to give out to customers. These include a standard track and trace form and a form to help you keep a record of regular customers.
